The FSHP Ambulatory Care Clinical Forum is dedicated to highlighting the invaluable role of ambulatory care pharmacists in improving patient outcomes and enhancing healthcare delivery.

This resource focuses on the significant impact these professionals have in managing chronic diseases, optimizing medication use, and providing accessible healthcare services.

In this resource guide you will find primary literature showcasing the value and/or impact of ambulatory care pharmacist on medication adherence, annual wellness visits, transitions of care and chronic disease states.

We also highlight the importance of collaborative practice agreements and pharmacist billing in enhancing the scope and efficiency of pharmacy services.
